Clinical Prescribing Pharmacists – Reference VC/CPP

Location: West Heath Medical Centre, 194-196 West Heath Road, Northfield, Birmingham B31 3HB

Salary: Depending on Experience

Hours: Range of sessions available 8am-2pm or 2pm-8pm Monday to Sunday

Duration: Initial pilot scheme November 2015 to October 2016 with the prospect for the service to be extended

Responsible to: Lead Clinical Prescribing Pharmacist (clinically), Service Manager (administratively)

Main Duties: Experienced Clinical NMP IP Pharmacist who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the primary care setting. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the service team to meet the needs of patients and supporting the delivery of policy and procedures.

Essential: Registered pharmacist prescriber with NPM Independent Pharmacist Prescribing qualification. Minimum of 3 years post registration experience with independent prescribing and pharmacist led triage experience as well as experience of clinical audits relevant to the service. Up to date knowledge of clinical skills required for non-medical prescribing and management of patients with complex needs, patient group directions and associated policy. Valid driving licence and access to a vehicle on a daily basis. Willingness to undertake a DBS check.

Desirable: Post Graduate Clinical Diploma or relevant pharmacist degree, mentoring or teaching qualification, clinical supervision training and experience.  At least 2 years recent primary or community pharmacy experience. Experience of management of patients with long term conditions. Knowledge of public health issues and the wider health economy.
